Partindo do conto «O alienista » de Machado de Assis, procuro tecer algumas consideraçāes sobre o 'normal' e o 'patológico' com refêrncia a Georges Canguilhem e à sua obra O normal e o patológico: ao procurar compreender o modo como a medicina estabelece o conceito de 'normal' e, em oposiçãāo, o de 'patológico', o seu pensamento foi influenciando um leque alargado de saberes que vāo desde a compreensāo da individualidade e a valonzaçāo da normatividade até ao conjunto das questōes relativas (...) ao trìnómio ética-política-psicologia. Neste artigo, reflicto, a partir da minha prática de patologista, sobre o sentido do diagnóstico morfológico e interrogo-me acerca de como um maior conhecimento das alterações morfológicas se relaciona com uma melhor compreensão das situaçōes patológicas. Sublinho a tensāo entre as dimensōes subjectiva e objectiva ao decidir, na incerteza, como é próprío do processo que leva ao diagnóstico. Defendo ainda a necessidade do raciocínio clínico para a razoabilidade de todo o diagnóstico. Starting from the novel "O alienista" by Machado de Assis, I aim at discussing on the "normal" and the "pathological", referring to Georges Canguilhem and his work The normal and the pathological. Trying to understand how medicine states the concept of "normal" and, as opposed to it, the concept of pathological, Canguilhems thought was influencing a wide range of knowledges, going from the understanding of the individuals and the valorisation of the normativity to the whole of questions concerning the trinomial ethics-politics-psychology. In this paper, I reflect, from my practice as pathologist, on the sense of the morphological diagnosis and I question myself how a dipper knowledge of morphological mutations relates to a better understanding of pathological situations. I highlight the tension between the subjective and objective dimension deciding with uncertainty, as it is peculiar of the process that leads to diagnosis. Also, I support the necessity of the clinical reasoning to the reasonability of every diagnosis. (shrink)

He never went through with it but continued to experience an intense desire to amputate his healthy limb for the following fifty-plus years. Described for the first time in 1977 as apotemnophilia, BIID was originally thought to be a psychological disorder characterized by sexual arousal associated with the desire to be an amputee. However, it was renamed when Dr. Michael First, a professor of clinical psychiatry at Columbia University, proposed that the primary motivation behind the condition was an issue of identity rather than of sexual desire.2 This proposal is controversial.3 Despite much [End Page 1003] debate, BIID was not included in the most recent edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), which was published on May 18, 2013.In this essay, we will consider the ethics of surgical interventions for BIID and Gender Dysphoria (GD).4 We begin with a discussion of the principle of totality that distinguishes ethical from unethical modifications of the body. We will refer specifically to the latter category of surgical interventions as "mutilations," though the Catholic moral tradition's use of the term is itself ambiguous.5 This is followed by an extensive moral discussion of (1) amputations of healthy limbs for patients struggling with BIID and (2) sex reassignment surgeries for those who identify themselves as transgender persons. I will propose that an ethical argument can be made to justify the former but not the latter.The Principle of TotalityBecause of the goodness of the human body, its integrity is a good that has to be protected and preserved. The ethical principle that is used to judge the morality of bodily interventions that impinge upon this bodily integrity is the principle of totality.6 It was articulated by St. Thomas Aquinas in his Summa theologiae as follows:Since a member is part of the whole human body, it is for the sake of the whole, as the imperfect for the perfect. Hence a member of the human body is to be disposed of according as it is expedient for the body.... If, however, the member be decayed and therefore a source of corruption to the whole body, then it is lawful with the consent [End Page 1004] of the owner of the member, to cut away the member for the welfare of the whole body.7Over the centuries, Catholic moral theologians appealed to the principle of totality to justify three different scenarios that involved the sacrifice of a bodily organ for the sake of the whole human organism. The first case involves a diseased organ that has to be removed to save the life of the soldier wounded on the battlefield—say a gangrenous, decaying limb as in the example of Aquinas. The second case involves the healthy foot of a man who is trapped on a railroad track that has to be amputated to save his life from a collision with a train, and the third case involves the healthy limb of an individual that has to be amputated in response to a tyrant who orders him to cut off his own hand or lose his head to the executioners instead. In these cases, the sacrifice of the part would be morally permitted as a necessary means of preserving the life of the individual. As Fr. Gerald Kelly, S.J., points out, this final case is particularly significant because it involves a healthy limb whose very presence—and not its diseased or trapped condition—becomes a source of vital harm for the organism.8How does one justify the principle of totality? What MacKinnon offers here is a provocative and original analysis of the meaning of the word "true." His applications are in the areas of statements in general, scientific theories, and theological propositions. One reason for the interest of the book can be found in MacKinnon’s intellectual odyssey. Setting out from a starting-point of standard neo-scholastic textbook philosophy and theology, MacKinnon has come to a highly personal synthesis to which he is willing at least tentatively to apply the label, "ontological pragmatism." (...) Stages along the way—and elements in the synthesis—include Bernard Lonergan’s transcendental Thomism, Wilfrid Sellars’ neo-Kantian version of analytical epistemology, and much detailed work of MacKinnon’s own in analytical philosophy of science. The provocative character of Truth and Expression is likely to bring criticism of the author for misuse of his sources. Wittgensteinian philosophical therapists are likely to bemoan MacKinnon’s relapse into ontologism, even if he qualifies it as a "pragmatic" version. Sellars is likely to object to being lumped with Lonergan, and the latter would probably be upset with the uses to which his former student has put his insights. It may also be worthwhile here to express a demur or two with respect to MacKinnon’s use of his Thomistic sources, e.g., concerning his theory of meaning. It seems that MacKinnon has accepted too naively what might best be called "textbook Thomism." It is true that Aquinas offers a mentalist view, and true also that both Aristotle and Aquinas speak of words as signs of concepts. While this can give rise to a private-language mentalism of the kind Wittgenstein would criticize, it must be recalled that Aristotle and Aquinas were writing not only before Wittgenstein but also before Kant. Whatever can be said of Aquinas’ "mentalism," it is certainly true that in other contexts Aquinas has a social theory of learning that is, at least on the surface, incompatible with a private language theory. Again, with respect to theological truth it seems likely that MacKinnon has been misled by Thomistic manuals, at least when he claims that what Aquinas offers is "a geometrically ordered deductive system" in which "the primacy of Jesus as a revelation of God does not fit." Historical studies of the function of his Summa Theologiae in Aquinas’ scheme for theological education do not support the view of Aquinas as a theological deductive system builder. Nor is it true, as MacKinnon seems to hold, that Aquinas’ view of theological truth is more Aristotelian than biblical. Aside from these qualifications on his use of sources, MacKinnon’s book offers an abundance of critical thinking, analytical insight, and systematic ability. For instance, the reader interested in MacKinnon’s original approach to an adequate epistemology of science will here, in chapter three of Truth and Expression, find the best summary yet of his many contributions in that area. Whether or not, in the last analysis, MacKinnon’s synthesis will be historically significant remains to be seen. What seems likely now is that if it does, the ingredient elements of Lonergen, [[sic]] Sellars, Wittgenstein, Quine, etc., will be more muted, and MacKinnon’s own synthesis will stand on its own. Lower respiratory tract infections are a leading cause of paediatric morbidity and mortality worldwide. Children in low-income countries are disproportionately affected. This is in large part due to limitations in healthcare resources and medical technologies. Mechanical ventilation can be a life-saving therapy for many children with acute respiratory failure. The scarcity of functioning ventilators in low-income countries results in countless preventable deaths. Some hospitals have attempted to adapt to this scarcity by using hand-bag ventilation, as either a bridge to a (...) mechanical ventilator, or until clinical improvement occurs rendering mechanical ventilation no longer necessary. In instances of hand-bag ventilation, an endotracheal tube is first placed. Family members are then asked to play the role of a ventilator, manually compressing a bag repeatedly to inflate the child’s lungs. This approach is fraught with numerous ethical challenges. A careful examination of the data and a nuanced approach to the ethical considerations are imperative. Ethical arguments in support of and in opposition to allowing parental hand-bag ventilation are explored, including the best interests of the child, the child’s right to an open future, beneficence and parental protection, legitimising substandard care, and finally, contextual concerns. An algorithmic, potentially ethically permissible approach to parental participation in manual ventilation is proposed. (shrink)

Although Sign Languages are gestural languages, the fact remains that some linguistic information can also be conveyed by spoken components as mouthing. Mouthing usually tend to reproduce the more relevant phonetic part of the equivalent spoken word matching with the manual sign. Therefore, one crucial issue in sign language is to understand whether mouthing is part of the signs themselves or not, and to which extent it contributes to the construction of signs meaning. Another question is to know whether mouthing (...) patterns constitute a phonological or a semantic cue in the lexical sign entry. This study aimed to investigate the role of mouthing on the processing of lexical signs in French Sign Language (LSF), according the type of bilingualism (intramodal vs. bimodal). For this purpose, a behavioral sign-picture lexical decision experiment was designed. Intramodal signers (native deaf adults) and Bimodal signers (fluent hearing adults) have to decide as fast as possible whether a picture matched with the sign seen just before. Five experimental conditions in which the pair sign-mouthing were congruent or incongruent were created. Our results showed a strong interference effect when the sign-mouthing matching was incongruent, reflected by higher error rates and lengthened reaction times compared with the congruent condition. This finding suggests that both groups of signers use the available lexical information contained in mouthing during accessing the sign meaning. In addition, deaf intramodal signers were strongly interfered than hearing bimodal signers. Taken together, our data indicate that mouthing is a determining factor in LSF lexical access, specifically in deaf signers. (shrink)

Using clinical examples throughout, Hoffman, Rice and Prout demonstrate that in many dysregulated children, _RFP-C_ can: Achieve symptomatic improvement and developmental maturation as a result of (...) gains in the ability to tolerate and metabolize painful emotions, by addressing the crucial underlying emotional component. Diminish the child’s use of aggression as the main coping device by allowing painful emotions to be mastered more effectively. Help to systematically address avoidance mechanisms, talking to the child about how their disruptive behavior helps them avoid painful emotions. Facilitate development of an awareness that painful emotions do not have to be so vigorously warded off, allowing the child to reach this implicit awareness within the relationship with the clinician, which can then be expanded to life situations at home and at school. This handbook is the first to provide a manualized, short-term dynamic approach to the externalizing behaviors of childhood, offering organizing framework and detailed descriptions of the processes involved in _RFP-C_. Supplying clinicians with a systematic individual psychotherapy as an alternative or complement to PMT, CBT and psychotropic medication, it also shifts focus away from simply helping parents manage their children’s misbehaviors. Significantly, the approach shows that clinical work with these children is compatible with understanding the children’s brain functioning, and posits that contemporary affect-oriented conceptualizations of defense mechanisms are theoretically similar to the neuroscience construct of implicit emotion regulation, promoting an interface between psychodynamics and contemporary academic psychiatry and psychology. Procedure manuals and textually mediated deathThe procedure manual as a document represents the practice of nursing care. Analysis of such manuals allows us to explore discourses of nursing and the ways in which they frame nursing practice. A critical analysis of a hospital procedure manual using discourse analysis was undertaken. A specific excerpt concerning ‘Last offices’ is used as an example of the institutionalisation of organisational values and beliefs as these influence nursing care. ‘Last offices’ directs nursing practices related to (...) the death of a patient, and the laying out as preparation for the funeral director or coroner. The analysis shows that procedure manuals reproduce hegemonic ideologies of nursing. The analysis also shows how nursing practices are constituted by intersecting forces of differing locations such as the hospital as an organisation, the healthcare system and the wider society within which these function. Pointing by apes is near-ubiquitous in captivity, yet rare in their natural habitats. This has implications for understanding both the ontogeny and heritability of pointing, conceived as a behavioral phenotype. The data suggest that the cognitive capacity for manual deixis was possessed by the last common ancestor of humans and the great apes. In this review, nonverbal reference is distinguished from symbolic reference. An operational definition of intentional communication is delineated, citing published or forthcoming examples for each of the defining (...) criteria from studies of manual gestures in apes. Claims that chimpanzees do not point amongst themselves or do not gesture declaratively are refuted with published examples. Links between pointing and cognitive milestones in other domains relating means to ends are discussed. Finally, an evolutionary scenario of pointing as an adaptation to changes in hominid development is briefly sketched. (shrink)

This manual is the product of a normative research program carried out over four years with Spanish-speaking populations in two geographically distinct regions: Madrid, Spain and the USA/Mexico border region. The manual describes a comprehensive system of procedures and normative data designed to assist the clinical researcher and the clinical practitioner in the neuropsychological assessment and diagnosis of adults whose main language is Spanish. Together the procedures comprise a brief and practical battery of eight tests for a basic examination of (...) attention, learning and memory, and executive function. The level of performance on these tests is assessed according to normative data. This is accomplished by adjusting test scores according to demographic criteria that simultaneously take into account the traditional factors of age and education, as well as the patient's place of origin. Cardinal Mercier’s Manual of Modern Scholastic Philosophy is a standard work, prepared at the Higher Institute of Philosophy, Louvain, mainly for the use of clerical students in Catholic Seminaries. Though undoubtedly elementary, it contains a clear, simple, and methodological exposition of the principles and problems of every department of philosophy, and its appeal is not to any particular class, but broadly human and universal. These all require a richer semantic picture than standard truth values in one static environment. In recent years, tech evangelists have made headlines predicting that in the future manual driving will be outlawed. This essay will investigate the question whether a ban of human driven cars can be defended on moral grounds in a future scenario in which autonomous cars are going to be significantly safer than manually driven cars. This article will argue that in such a future scenario manually driven cars, for moral reasons, indeed should be banned from participating in regular traffic. Since (...) the moral argument for outlawing manually driven cars will likely be met by resistance by car-aficionados, in the final part of the paper, we are devising a proposal for reconciling the strong moral case for a ban of manually driven cars with the widespread fondness of manual driving. Israel Gottlieb Canz’ im Jahre 1742 erschienene Theologia naturalis thetico-polemica ist die erste von drei Abhandlungen dieses Autors über natürliche Theologie. Diese ist bei ihm als eine Art Vorbereitung für die „theologia sublimior“ (die geoffenbarte Theologie) zu verstehen. Die Theologia naturalis hat bei Canz als eine theologia thetica eine darlegende und beweisende Funktion. Dahinter steht eine programmatische These, nämlich dass sie als alleinige Erlöserin des Menschen von seinen Sünden nicht ausreicht. Vielmehr soll durch sie die Notwendigkeit des Weges zur Versöhnung (...) aufgezeigt werden; den Weg selber zu erforschen steht ihr nicht zu. Canz’ natürliche Theologie ist auch eine theologia polemica. Gleich in der Praefatio präzisiert der Verfasser, dass er die Systeme der Gegner, ihre Argumente usw. darlegen und sie analysieren und diskutieren werde. Dies erfolgt aber im Hinblick auf den eigenen Diskurs bzw. im Ausgang von diesem, der sich in Konformität mit der geoffenbarten Theologie weiß. Canz gehört zur ersten Generation „Wolffscher Theologen“, bewahrt sich aber eine gewisse Unabhängigkeit gegenüber Topoi der Wolffschen Philosophie und geht durchaus eigene Wege. Dies im Einzelnen nachzuweisen, gehört zu den zahlreichen Forschungsdesideraten im Hinblick auf die Philosophie des 18. Jahrhunderts. Israel Gottlieb Canz’s Theologia naturalis thetico-polemica, published in 1742 is the first of three works by this author on natural theology, which in his case should be understood as a kind of preparation for the ‘theologia sublimior’ (revealed theology). In Canz’s work, theologia naturalis has the expository and substantiating function of a theologia thetica. Behind this stands a programmatic thesis, namely that theology alone is not sufficient to redeem mankind from its sins. Instead, the necessity of a path to reconciliation must be demonstrated; it is not the role of theology itself to seek that path. Canz’s natural theology is also a theologia polemica. In the preface the author states that he will explain, analyse and discuss his opponents’ systems, arguments etc. However, he does this with reference to his own discourse, or rather taking that discourse, which is recognised as conforming with revealed theology, as a starting point.
